Just a suggestion, try getting a mechanics basic toolkit. It doesn't inspire people when you are using joinery tools, claw hammer, etc.Also it's easier to do the task, with the proper tools. The USA has always had an advantage, and the best tools are available at the lowest cost, unlike the rip off uk. The bent washer, is a locking washer, by the way.

The bent washer would be intentional, the washer hole will have a shape to match the shaft, usually flat sided so that the washer will lok to the shaft and stop rotaion.
After the nut is tightenred the edges of the washer are bent up to lock the nut against undoing.
Removal of the nut is usually after using a chisel to flatten the washer.
