That's not to say that if she's feeling ill or tired, she should come to him. Both the husband & wife are to satisfy each other needs, as they could not look to others for social relations. So if a wife were to intentionally avoid her husband so that he can't have any pleasures, then she's responsible.

It means that a woman who in the worst degree, is being abusive to her husband, to which if all fails, such as admonishment & separation of the bed, then a husband can only hit his wife as the Prophet Muhammmad (PBUH) said using a toothbrush. Also, if you had read verse 4:19, you would've known that the verse informs muslim males to treat women with respect & dignity. So it's obvious that you're not being accurate.

Women in Saudi Arabia may not be able to drive, their right is restricted. But they don't get beaten up or killed the way how women do here in the United States. That tells you that women are more protected over there than here despite facing injustice.
