Aaron and I were lucky with this; we didn't have to give specific documentation. We were able to find some, though. Here's what we did: Check your bank records for payments to any weight loss stuff: gym memberships, etc. If you've changed banks, the gym or whatever will have records. Also, your weight history should be available to you from the OB/GYN. Also, I totally agree with Tom re: knowing your coverage rights. Go Tom!! (He Rocks)
I'm pre-op, but can give some advise on this area. Go to your insurance website and download your "Certificate of Coverage". This is your bible and is your employers contract with the insurance company. If the insurance company does pay for WLS, your employer may have an exclusion. This paper will lay it all out for you as to what is required if covered. And I agree with DT ... Ask for copies for you of EVERY TEST. Keep it. The docs always say they didn't get something. See below.
This way you have a copy and can provide it without trying to get another copy from the testing facility. Another common comorbity is Sleep Apnea. Are you tired during the day? Do you snore? These are common symptoms and you might have it. First step is finding out what your insurance company requires and if you have coverage for WLS. Then you can make a plan of attack. It may take a while to get everything in order and meet their requirements, but it will pay off in the end. Tom

Then, I found youtube and just recently been researching it more and more since January. I can't believe how great it is to have people like you that are willing to share your experiences.
