I don't think this song is actully about harming your self. I think its about trying to help someone self harming, being the friend or confidant of someone thats hurting themselves. 'cause it's hard and somedays you come so close to hurting yourself because you "care too much" but you have to hide it so they don't hurt themselves. You have to "sew yourself shut.
