This happens mostly because of server lag (obvious) and the fact that spy is chasing engie, while he runs backwards; just enough where the backstab goes through the front and into has back anyway. (Not so obvious)

I learned the best way to kill back stabbing spies is to stand completely still and swing your melee weapon at them while they try to get behind you for the stab. You'll eventually hit them and they are usually too retarded to switch to their gun.
