  • During the interrogation did the engineer NOT see himself digging his own grave? Great story of multiple elements. Thanks for the video, Karaniskool40.

  • A good design would be if the bolts were on the inside and outside :D

  • who put in the first set of wrong bolts. must have being the plane maker, but hey lets blame aircraft engineer, stich him up the poor mug, thats how big buisiness tells the goverment how to do there job, more lobbyist money for right honorable men in power, job done everyone go's home happy apart from joe bloggs, yeah nice one. pilot hero's my arse, there the ones spraying chemtrails all over the world now, poisoning the planet. wake up you sheeple.

  • this is the real part 6 watch?v=XVOpaDEIEcg, skipped to 41st minute

  • One other factor is that this happened in the morning light rather than in the dark night which wud have made the situtions more worse for the co pilot and the crew.

  • On my god. This is unbelievable. The crew was absolutely courageous. Tim's survival was a miracle. It is a pity that Alistair chose not to participate in the program. It would be nice to hear his perspective.

  • humans make mistakes, we shouldn't blame people very easily, the bolts fitted before were also not the correct ones!!!!!

  • dumbass mort had to be a idiot.

  • British Airways- Best pilots, worst management.

  • Matched by eye!?!?!? WTF It's peoples life at stake!!! He should be jailed!!!!! ignoring the advise!!! NONSENSE

  • @zareena1985 not going that far that would be a dick move i think the people that designed the window should be fined

  • @barf245 The guy that fixed the window was fined for 150,000 dollars and was jailed for about a year.

BY THE WAY this is the last part of the episode there is no part 6.

  • where's the end?

  • Well done to the co pilot for his heroic saviour. And well done to the other crew members who help saved Captain Tim. Glad your okay Tim.

  • Does anyone know if there is an episode of this program on the investigation to the "Value-Jet" in the Florida Everglades several years ago?...I remember it happend On or around Mothers day (don't recall what year). That crash really shocked me & has been on my mind ever since! I was working construction in Miami near the airport at that time & Seen the Flight Take off, then heard about the crash on my radio on the way home... I'd like to see exactly what happend on that flight that day.

  • Is there a Part 6? The announcer seems to stop in mid-sentence here.

  • This is the same dammed chain of events that put to few spacers around the outside of the drill bore casing because in past they had gotten away with it and all slapped each other on the back for all the production time saved by not putting in enough spacer. These fools are doing same dammed thing with atomic steam engines, stuffing way to many atomic piles in the vessels then the control rods can hold back. The DOD is doing same dam thing with 60 thousand super atomic bombs pointing everything.

  • Are you all getting this. I would not be surprized if that same man has something to do with the BP blowout in the gulf of mexico. And if not him his genetic cousin. I know what he did. The install before him had cheated and put in a FEW undersize bolts because some of the holes had gotten cross treaded. The install before him might have even used some kind of shimming compound to cheat further on the install. This guy coming behind the first screw up went ahead matched to the cheater bolts.

  • That's 'Brutish Airways' for you - cattle cars are for making money and safety maintenance procedures be dammed. Good crew!

  • "Not by the book... As you would train someone new?"

That is a problem in EVERY repair or installation of ANYTHING.

Be it an electrician or an asphalt paver.

  • did pilot not have seat belts on?

  • the last bit is cut off :(

  • does anyone else think it's ironic that the investigator's case was thrown out, because they did not follow procedure.... when their findings where the engineer did not follow procedure???

  • @nmhamilton7 and thats why the law needs to be changed!

  • @QuantumQuacks Thanks, I was looking for the rest of the video.

  • Why did the engineer ignore the Maintenance Manual and go directly to parts store?

    Answer = Complacency

  • People create so many different types of bolts that eventually nobody knows where they fit.

  • @Hobu123 Yeh ,lets go to one size fits all to eliminate the bolt confusion.

Too small? just wrap tin foil around the bastard.!

It works on my windshield wipers and I drive at 90 mph.

  • @jonesgerard LOL Hey I'm curious. Do you have indoor plumbing where you live yet?

  • @jonesgerard Your windshield wipers suffer a tad less stress than the window of a damn airplane.

  • brummy accent! classic

  • No Airworthiness

  • Very delicate stuff. There shdnt be any hint of assumption.

  • I'd have thought the engineer would've have felt the bolt's thread wasn't mating properly.

  • NEED PART ^ NOW

  • Wow.

It would be easy to shake our fingers in judgement. but the Engineer replaced the bolts that held the windscreen in for FOUR years. When you consider the amount of aluminum that BA puts in the air each day, successfully, that is one very well run airline.

Karaniskool, thank you so much for putting this series up. I NEVER saw them here in the states. My brother flies Gulfstream G4s and I am always interested in how things work I greatly appreciate your time!!

  • What happen to part 6? Where is end of the show??

  • complacency..

  • I bet everyone thats watched this video are pro engineers.

  • You can see the end part here:

    /watch?v=--ORTEkxRfA

  • They should have British Airways flashlights

  • What sucks here is the fact that the person who runs this doesnt answer peoples questions as to where is PART 6!!!

I wont watch anymore of your videos and I'll do nothing but give bad remarks now.

To let people watch without ALL the info is just plain BS.

    Enough said?

    Thanks

PS, Any and all links or attemps at links to part 6 by other fellow wathers (and thanks for trying too~) dont work.....

  • Wow Im an AME student and this is just ridiculous

  • once again dirty, robbing, raping Lawyers dictating outcomes??? when it is clear to anybody with an open mind that these guys did a great job in finding numerous faults and bad procedures, yet these fucking dirty raping Lawyers just had to fuck things up as usual and again,i hope non of your loved ones fly in these planes on the strengh of what these particular Lawyers have had to say FUCK!!!

  • it's only because the engineer rushed his work

  • Stupid ass engineer

  • I think it is good to do all this checks on the engineer but any layman can see that the real problem is in the design of that window. A similar mistake in the baggage door of the DC-10 was, probably, the main cause of the end of that manufacturer.

  • My friend, daneil cassidy was the engineer that "fixed" the window. He often uses lego to repair planes. He has recently enrolled as a pilot at "onur air".

  • Don't you love lawyers. They quashed a safety report on a "technicality". Maybe if a lawyer is on a flight the pilot should hold him (the lawyer) out the window.

  • That plane was a piece of shit, I mean who the hell would buy a plane with not enough oxygen masks for the passengers and also with no fuel empty thing

  • Part 6 is on takeofftube[.] com

    ADD whats under.

    /view/1598/air_crash_investigation_ripped_from_the_cockpit_part_6/

  • @dreamshooter90 no such beast of take off you tube.com

  • Part 6: takeofftube com/view/1598/air_crash_investigation_ripped_from_the_cockpit_part_6/

  • @pixelplayground don't forget to add the dot before the com, link posting is frowned upon...

  • Part 6? or maybe the whole thing on another video service that doesn't have a short limit?

  • i hope all old aircraft like these have been recycled. it's no better than scrap metal.

  • Gutted that I'm unable to watch part 6 of this and find out the final results of the investigation :(

Was pretty shocking how Tim managed to survive when he was outside the plane for so long! Glad that everyone on board survived the flight too, including the pilot!

  • pretty disappointing when you watch all 5 parts only to find that the last one is missing...it's like doing a jigsaw and finding that you're missing a piece :-(

  • Where is part 6?

  • @vallorahn there isnt one

  • Where the f@#$ is part 6?

  • aw. I would have loved to see part 6 and the changes induced by that final report...

  • Where is part 6

  • Where is part 6??????

  • Unfortunatly this is a common problem made worse by pressures to make things faster and cheaper.

  • Many times the parts recieved from supliers are in error and it is the technicians job to notice this and report it. Sorry to say that in this case, this practice caused the tech (engineer in UK) to believe he knew more about the job than he did. You ALWAYS use the documents to verify parts. You never just put back what was already installed. That would bypass updates in documents and past practice corrections.

  • I work as a technician on military grade equipment (sorry this is as specific as possible) and I know how many details of assembly and overhauls are left to the experience of the technician.

  • Now imagine a situation that you start developing a plane right out of nothing. Would it come to your mind to put the screws from the INSIDE? Millions and millions of these tiny little details had to be upgraded through years to form an extremely sophisticated piece of technology.

  • where is the last part?

  • Why not colour code the bolts like resistors, theres no particular reason for them to all be silver is there?

  • But he should of listened to the store manager!!!! So park a plane well for maintenance, read the manual, and listen to the store manager!!!1

  • Hmm sounds like it's both this engineers fault and the past engineers fault.. one for putting in the wrong bolts in the first place.. the other for not checking the manual when he should of.. kind of a shared blame here...

  • I was on a BA1-11 British Island Airways flight from Sardinia to Gatwick 0n night of 17th Oct 1988. In turbulance and At 44,000ft we had a cabin depresurisation had to go on oxygen had to make a rapid dissent. Never told cause of problem. Has anybody got any info of know where I can find out details please. Thanks Trav

  • @000CLASH000 The engine blew up and you're in heaven right now. I'm sorry.

  • Its incredible that bolts of 200ths of an inch could cause a massive accident that nearly killed a poor pilot! He should have taken the advise and chosen the right bolts!

  • Stupid design of the windscreen. The bolts should be attached from inside the cockpit. there is no chance of them ripping off

  • @MrPauly2010 Well done, this was said in the clip!

  • @MrPauly2010 Listened carefully? There are 2 types of WINDSCREEN - inside and outside mounted. How do you access to the bolthead from the inside of a plane anyway?

  • @MrPauly2010 altho you are correct...you have no idea... how hard it is to change that thing -_-

  • @MrPauly2010 Disaster is due to use of bolts with bolts with slightly different pitch angle..!! All the Forces and Stresses will be considered while designing, you can't claim that if bolts are attached from inside these kind of catastrophe will be avoided, there may not be equally significant dangers, but failures exist at some different circumstances

  • @MrPauly2010 It's not a matter of which direction the bolts are installed. It is a matter of whether the wind screen is installed inside of the frame or outside of the frame. Proper bolts = no problem. Wrong bolts = boom!

  • @460823357 exactly i fly in flight simulators but i've been told by professional pilot's online that i should go to flight school as i know all the aircraft i fly like the back of my hand and i can operate them professionally. Some people like flying the aircraft using the outside view but i'm addicted to flying in the virtual cockpit as that's the exact cockpit of whatever aircraft i operate at that time and i can control everything. The only time i use the outside view is when i'm parked :D.

  • @460823357 Also I've even impressed southwest airlines air traffic controllers as well as many other ATC personnel.Even know how to commuicate with other pilots & air traffic controllers just like i was flying the real aircraft instead of a simulated model. My reason for not going to an professional flight school is simply because i lack the funds to pay for it.But until the day comes my income increases enough to where i can do what i really love i'll just enjoy simulators.

  • @460823357 Now even i've had to fly a aircraft with massive failure and yes you can set the failures to happen but in my cause i hadn't and lost engine 1 & 2 while flying a delta 737 and was 53km from the nearest airport and was at the time at a altitude of 32,000ft so you see my problem bu i managed to land the aircraft without incident.I haven't had to many failures in the aircraft i fly in the flightsims i play but once on takeout all i'll say if goddamn bird.

  • @bkeener86 What a pathetic comment.

  • @Homerard yeah patheic because i'm not sick like you and i actually feel sorry for those involved while your likely so bitchass child who thinks people dying is cool so fuck off kid we don't have time for your bullshit. To top it off fool i said i fly for delta airlines but also fly in flightsimulators as well.I doubt you've ever piloted a aircraft with critical failure's nope your likely a smartass kid that think's he/she knows everything. And yes once in a flightsim i had a failure due to a bird

  • @bkeener86 Sorry disregard last comment!!! It was meant for another comment. So sorry please just ignore it/flag for spam >.<

  • Where is part 6 ?

  • @cuscocrew its the missin window u idiot!!!!!

  • Blame it all on the engineer? I would blame it on industrial designers who produce bolts that differ in size only one or two hundredth of an inch. There is no sane reason for this. They should have designed bolts that differ in size so much that no one can make a mistake.

  • so how many size bolts do you think we need for ever machine in the world?

  • where is part 6 - was it deleted? was it stuck on with the wrong size bolts?

  • honestly i think the guy who replaced the windscreen should be prosecuted

  • What doesn't kill you makes you stronger..

  • i think u're a slipknot fan :)

  • @Nafeillom unless its a non terminal muscle wasting disease

  • ALSO....No mention was made of torqe tightness, screw locking (locktite, say),or posi.head ,not slotted head.

  • SlashRevOlver...8 mths ago,

The bolts were not only undersize, but a diameter, say 6mm (it looked like) too short. The fitter should have been shot!

  • thank you

  • Part six at: takeofftube com

    /view/1598/a/

  • So how did the wrong size bolts get in there in the first place? Before this engineer?

  • noladol 1mth ago......Because the fitter prior to this fitter was just as big a conceited dick head as this one.

Retired Bae standard's engineer.Fitters think they know better than 'the book'

  • Part 6? What happened? Did it fly out the window along with the documents and the pilot?

    kp

  • Christ if human factors was used on me the guy would need a toilet roll to write it all on hahahaha

  • part 6 flew out the window

  • ...with pressure from BA lawyers I suppose

  • haha

  • It was a stupid decision, but I can understand why he made it. After 4 years of operation with identical bolts I would likely make the same mistake.

  • well you just look at the plane and think to yourself.."this is scheduled to take 80+ passengers and 6 crew overseas to holiday...shall i be risky and eye match the balts early in morning, or do the job as i was trained to!!!!!"

  • part 6 missing?

  • part six?

  • where is part 6??

  • 2 kind of the same episodes ?? why ??? where is part 6 ?????? happy that the pilot survived and the co-pilot is above outstanding and deserve alot of awards !!!

  • I can't believe the pilot survived...absolutely incredible!!!

  • this is a serious mistake about an hardware engineering maintenance. here's my conclusion.

  • oh well, this episode was getting a bit long anyways.

  • What a f*** bullshit with these terms of use, copyright shit GEMA and all that crap...

5 Parts okay and the 6th deleted because od terms if use violation? All the film Indytries and all it´s belongings are laughing at us...

    They are joking us...

  • I tried to find part six and was informed by YT that the vid. had been removed for terms of use violation.

  • karaniskool40...where is part 6? so I was intrigued watching the previous parts and I can't find part # 6???

other people are asking for part 6 and you don't give any answer.??..wtf!

  • Yeh wheres part 6?

  • I couldn't find a part 6 although the episode isn't finnished at the end of this. Where is the end?

  • that engineer did a botch job and was just plain lazy

  • Comment removed

  • amazing how precise everything needs to be in aircraft, 1/200th of an inch difference in screw diameter could have crashed plane if co-pilot was not strapped in

  • Slashrev0lver: I'm pretty sure the video meant 2/100th or 1/50th inch. But you're right, it could have been catastrophic.

  • Comment removed

  • @SlashRev0lver ok, so i am tired...didn't read your post very well the first time through ;)
