You can get pretty close with some fuzz pedals that are out there now. Neils using an octave pedal somewhere in his rig though and has done with this song for some time now, i can get fairly close with a big muff pi usa version and the whammy pedal octave setting.

Rust Never Sleeps/Live Rust - have both an acoustic version and electric version which was very similar to the version played here (kindof like what he did with Rocking in the Free World on the Freedom album in the late 80s). Both songs earned Neil the title "Godfather of Grunge" although I wouldn't say he alone (or either song) was as instrumental to grunge music as people assume - Rock and Roll will never die!
