I think these are superb guidelines, however, David does not suggest anything practical how to reduce the cost at the level of the primary transaction - patient to doctor. under current system patients can get any procedure they wish regardless of the cost. there must be some incentive for the patient to reduce the costs of procedures, otherwise we will not get anywhere.
