When I went through Educator Space Camp in Huntsville, AL, they told us that they had only ever had one person get "sick" on this, and that person was already complaining of being "sick" when they woke up that morning (from an illness) but got on this, anyway. This is designed to keep your center of gravity in the same spot, no matter how everything turns. So, people don't get sick on it.
This device is known as the MASTIF, the Multiple Axis Space Training Inertial Facility. What it does is simulate a space capsule spinning out of control in all three axes and the astronaut would have a stick to try to bring it back under control. I don't think they use it anymore and I've heard of people getting sick in these things but nobody's ever died that I've heard of.

thanks for asking! i don't think people have died on it, there were no warnings, disclaimers, waivers, etc. PLUS, this is not meant to simulate zero-gravity. it was used during the capsule era (not anymore) to simulate loss of attitude control, where the capsule could start spinning very rapidly. it did happen at least once for real.
Have people ever died on that, PLUS, how does this simulate space because there is no reference of gravity in space so how does this reflect for zero-gravity environment?
