@xXJOEPOTXx when the enemy charges toward ya, just press right bumper. and there's V.A.T.S. It helps to just randomly press it when ur out in the wastes, too. It helps you spot threats

When you go into VATS you can select body parts using a gun. or just the entire body when using melee or unarmed weapons. After selecting what you want to attack hit right trigger, that will select it to be attacked, you can select multiple targets at once. Once your finished selecting your targets press 'A' and then your attack will commence.
