I'm not sure about the Fed purchasing government debt example...I think this is the exception rather than the rule, I believe the debt is sold at auction to many buyers world-wide...
and yes, the money is created from debt...basically, the debt is issued based on the taxing capability of government, which makes sense to an extent but could get out of control...
