well depending on the name Omerta...it's very clear i guess that it's about a guy that broke the Omerta and went away and hide with a new name and a new family.

But then an old mafia friend comes in (probably his real brother or he just calls him brother) and thats when he realizes that they found him and wonders why it took so long but he knows he will die soon. he remembers old times and suddenly becomes aware that he was already poisoned speaks his last words and dies.

It seems to me like its a song written after death..think about it, he doesnt know who he is anymore..he's a ghost, talkin to people at his funeral, "why have u waited so long", "long since i saw u so how have u been" meaning why havent i seen u till now? long since they told u that they found him (him bein himself dead), he can see them, but they cant see him, thats why he's happy his brother came, cause he was close to him..its much deeper than it seems..
