When Sam fell in that glass it was laying on the ground but when he brought his hand back up, it looks like that whole chunk of glass when into his hand because the glass wasn't there when he brought his hand up, just his blood. It looks like it took a good chunk out of his hand and that's why it's bandaged. Probably required stitches.
