I"m stuck with i-134, and 1040 forms, I won in the Diversity program 2012, and passed my interview succesfully on November 14, 2011 at the US consulate general in Casablanca, my visa is approuved but it's suspended untill I bring i-134 and 1040 forms, nobody in the US wanna help me, I don't wanna miss this chance, if anyone can help me I'll pay 1000$ for these 4 damn pages. I can support myself and find an appartement I just need these forms to liberate my visa.

If I am right, I guess the Affidavit of Support form is needed by all those people who have family-based and some employment based immigrants. This is aimed to show the there is an adequate means of support after immigration to the U.S.
Further I wish to add here is that people are looking for family-based immigrant visas, including certain orphans must present an US Affidavit of Support form. Similarly, all those applicants who are for employment-based immigrant visas where a relative filed the immigrant visa petition have to put forward the Form I-864.
