Her sister Madhur told that the chains are real chains. And they were really heavy. The director also didn't want Madhubala to eat anything the whole day, so that the pain would show clearly on her face... really interesting. Love this song, so much pain...

Um...did you get that from the show Yaadein hosted by Pratibha Advani? Yes I agree with you about the real chains and the starvation, because K. Asif was some sort of perfectionist like Stanley Kubrick when it comes to directing movies and he wanted everything to be as realistic as possible...in short he was obsessed with realism so much so that when Madhubala's father begged him to remove the chains because it bruised Madhubala's body all over, he said "no"...
