Pyramid Head is a representation of James, and all that he hates about himself.

Originally, he killed Mary because a) he really DIDN'T want to see her suffer, but also b) because she made him unhappy when she was sick. He was very sexually repressed, and felt.. obligated to stay with her; though at the same time, he still deeply loved her.

When he killed her, he felt so much guilt for doing it, that he created a reason to go to Silent Hill.
