prima mean cousin as a first meaning......Calling
any boy or girl primo or prima is just a slang.....
In Spain they also have the slang custom of calling everyone uncle or aunt......( TIO or TIA )

Silvia is the sister of Lola, Paco's (ex) wife. Sara is Paco and Lola's daughter and the neice of both Pepa and Silvia.
