Examining the Knife gets you the bad ending. You have to resist gamer instinct the whole time, and never look at the damn thing to get the Good+ ending.

@drakesyn You get the in water ending if you examine it constantly, but you also have to stay at low health for most of the game, and be really slow to heal yourself.

There are other factors.

You should examine everything at least once. Even the Maria ending is more dependent on keeping Maria from getting hurt than examining the letter and photo.
