I can tell you that I go from love to hate and back with Clarkson and the only constant is that most of what he says of the US, is true to some degree.

What was the hospital sequence about? Hammond's accident? Wow, that would have been a loss to all of us.

The hospital sequence is a collection of pics from TV programmes James May and Richard Hammond have appeared in. The pics of James in scrubs was of him viewing a heart operation to demonstrate 20th century technology in medicine and the human body (TV programme "James May's 20th Century").
