well, I Can't really translate the whole song, but basically, the world is ending and Luka is singing her last song to "you" in the song. Then she dies/disappears. "Hello, Planet" is a song that is connected to this song, but nobody really knows what happened in between those two songs. Some people think that Luka died and left a flowerpot, while some people think that Luka merged herself with Miku, and Miku took care of the potted plant and saw "you" from Wonder Last.
