@jack04083---it does remove skin. It's a hollow piercing needle. I've done suspensions with the two featured in this clip, so I can say first hand, you lose flesh lol.

There's a difference between a punch and a needle. A needle doesn't remove the skin, it just cuts/pierces a wider area/hole and immediately stretches that hole to the thickness (gauge) of the needle as the needle is pushed the rest of the way through. A punch removes the skin.
Otherwise, pretty interesting video. Looking to do my first suspension sometime soon.

When using relaxation, meditation & breath techniques and good piercing techniques can getting pierced at a very low painlevel. ( 5mm needle & hooks)
After 20 minutes some pains occurs, relax & some minutes later by new endorphins.is created. I knew this already my 1st suspension I hung 60 min. I hung > 90 min. the last time.
@lighthouseprj your body can, yes, but you wouldn't want to keep hanging through multiple cycles; you don't want to train your body out of its natural rhythm or even make yourself "more able" to handle it longer/more extreme/etc... because of the endocrine changes that occur, regular practice of this can make the body lose a glorious human ability to feel extremely subtle, nuanced sensation; this heightens you in the same way really hard masturbation does,making regular touch insufficient.
@lighthouseprj and I'm not against suspensions, to be clear--being in medicine doesn't make me against a lot of the things people stereotype us as being (I'm not a pharm pusher, either--it's a last resort for 90% of people apart from those with actual glandular/organ malfunction). This should be in extreme moderation as we don't WANT to train a delayed/insufficient response should REAL danger be present, same with burning oneself, etc. & while you may not notice the desensitization, lovers will!
Q: has she developed a higher pain tolerance for getting pierced?
A: Nope. It hurts every time lol.
