How do you force the insurers to compete on cost and quality? Why not just eliminate them? What happens when people lose their jobs (which often happens concurrently with being sick). Portability is key, and shouldn't be tied to being employed. Single payer universal coverage (where there is but one enrollment per lifetime which eliminates huge costs right there) is really the only solution if we are to join the 21st century in providing comprehensive health care access in this country.
