You didn't SEE him reload, but there were several seconds where he was off-camera and the shot was lingering on the Deadite. There was also a chink-chink noise audible on the soundtrack. I therefore submit that he put another shell in during this period.

You can hear a pump. This is a double barrel shotgun. Inorder to reload it you need to open the barrel and take the bullets out before putting them back in and putting the barrel back into place. Bruce does this physically many times in Evil Dead 2
