How do they blame the chart saying it was faulty when he shouldnt have been at that altitude at that point? For clarity, the charts are planned so that only pertinent info is shown and ground that is so high that it poses a threat. Air crash investigation is a good series but sometimes they seem to cut corners.

One of the worst aircraft disasters I have ever seen.  ATC was in clear disconnect. An incomplete approach chart? Not. Let the flight proceed. We all agree the pilot failed.. Pilots rely on reliable info from ATC, but they have the final decision. They have the information on board. The pilots ignored minimums.
