keep in mind there's more than 1 dialect in the Japanese language Mi is the representation for demon. It's the character name for demon. did you even watch the series? in Higurashi no Naku Koro ni Kai it mentions that since Keiichi didn't give Mion the teddy bear, she cried in front of Shion... Shion then jailed up Mion, masqueraded as Mion a bit and killed some ppl. Mion mighta been the one that injected Keiichi early on, but Shion's the one that killed while wearing that white robe.

It's pretty easy to tell their actions apart, because when I watched the episode when it seemed like Mion was going around killing everyone, I just couldn't see her as the person to do that, so I never believed it was Mion. Though I could barely tell who it was because it was too confusing.
