@coralinejonesish the old lady lives in the same house, but it's split up - in the book there is 3 floors, with one apartment on each floor.

in the book, she covers herself in paint because star hates her tattoos, she thinks if she paints over them then Star will like her again and come home.

I haven't read the book in years, I'm not sure if it covers her condition, as the book is told from Dolphin's point of view, and I don't think she has been told, as she's a child. You should read it (:
