The click death of Zip drives was eventually discovered to have been caused by a manufacturing defect.
A small foam washer was omitted from production to save costs. The purpose of this washer was to cushion the head assembly when it hit the end stop. Without this washer the heads would be knocked out of alignment, causing the click death.
