The phaser is Mu-tron bi-phase. These usually have a blue or purple finish but I think came with white sheet that could be laid over showing preset settings. In this vid he is using the Super Phasing sheet but there were others like stereo and two speed.

@MisterPinkNoTip It's probably a Mutron. Lee was always using the Mutron. I have never seen one like the phaser in this clip though.
Everytime I watch this clip, I'm shivering with good vibes. Too bad they never released that track.
