She said when she looks at the chart, the chart need to reflect the PE was thought about and ruled out with pertinent negatives and positives.
There is a huge distance between negligence and judgement. One says, I didn't ever look at how bad things can go. The other says, I looked. Weighed the information, and made a decision. PERC rules, I believe, screams "I looked." Peace. Feedback encouraged.
