That is an montgomery wards evergo original. it has the same green sticker that my evergo floral hugger has. it says something like turn the fan on high before turning it on low for better performance. it also has the same green noisy motor as my evergo floral hugger. Very cool and uncommon fan!
I could replace the capasitor and correct the wiring on the reverse switch
