After all this Big Boss went into the Vietnam war he fell into a coma after a battle and the injury rendered him sterile, the U.S goverment not wanting to lose the legacy of the greatest warrior o the 20th century decided to clone him, he wasn't aware of it because he was in a coma but i believe Para-Medic(Dr. Clark) was in charge of his cloning.
