There's an extension called Autohide that allows you to go full screen without hiding the toolbars.
Also, the Stylish extension is great for making Firefox more compact. There are some useful styles on the userstyles website, but if you know CSS then you can really customize it to your liking.
As for pressing F11 - the reason you don't get buttons is because you moved them into the top row. Me, I just have them as small icons (I keep bookmarks where you placed the buttons) and they show up when you move your cursor to the top of the screen. Having your taskbar to auto-hide is also nice in combination with F11 as you really maximize the space.
screens are getting smaller and my eyes are not getting better

Very nice!! You can also use the Autohide extension, wich lets you configure the fullscreen mode.

Also a nice thing for the people that don't like the "New Tab" button on the top is to press CTRL + T and it will always open a new tab.

Another thing you can do - Auto hide the task bar, then hover over it when you want to do something... Simple

Just hit F11
Also use any of the compact themes for Firefox. My favorite is hmmXP for Firefox 3, or Classic Compact.

Can you place the back/forward etc. buttons before the File menu? That feels more "natural" I think.

Yes, you can. :)

You could also try the tinymenu add-on to reduce the space taken up on the left by the menu buttons.
