Ven's heart rests inside Sora after a certain part (thus cutscene) of the game. When that happened, the faceless Vanitas ended up looking like the later grown-up Sora, but a darker version. Thats why he looks like Sora, simply put.

If you don't know about it already, stop reading now and ignore it please.* Ventus only disappeared because he defeated Vanitas, who was the dark part of his heart, so when he beat Vanitas and he disappeared, Ventus disappeared to.

The X-Blade doesn't exactly represent anyone, it is merely the Keyblade used to release the powers of Kingdom hearts and unlock the secrets of the Keyblade Wars.

I remember from Xehanort's report the the X-Blade is created when a person of pure light and a person of pure darkness, who are equally powerful, crosses (as in their hearts intersect) the X-Blade is created.

Now for why Ven disappeared (but not gone), it's simply because he literally destroy his darkness; Vanitas. With the exception of the 7 princesses of heart, every being in the KH universe are composed of light and darkness.
