actually ur form is really good and your head should be in as the same angle as ur back, not cranked up like people think, head same angle as the back so slanted down somewhat but ur eyes should be up and squating outside the rack is fine, either have a spotter, or know how to dump the weight properly without a rack, i step out when im ramping up to working weight and get all the way down with my squat, then my working weight i squat in the rack and get as low as i can

You are doing nothing dangerous. Your knees are safe. You now need to work on keeping your back upright. Look up and keep your abs tight and upper back straight.
pretty good, my biggest tip would be to look directly upwards during your squat. you can see, especially at the top of your squat, that your back is kind of bending forwards, try looking directly upwards, that should correct it. that said its still good form

look up
ur upper back is lose. it should be tight and locked into place. pulling the bar into ur traps
looking up - will help keep the chest up when going heavy. the body will follow the head/eyes
