In all the years -- since 1994 -- that I've played this game, I've never seen Alexander turn into a beast except in videos on the Internet. When I've played the game, I've always done the necessary things.

What exactly is it, in gameplay terms, that results in Alexander turning into a beast?  Is there a time limit, or is it triggered by Alexander doing certain things, like going to the location with Beauty's house without all the necessary items?

I always thought the catacombs were the scariest part of the game, even more than the land of the dead. For best effect, play at night in the dark. Ah, brings back memories. But some of these ways to die even I didn't know about!

Wow. I forgot how ridiculously unthreatening the minotaur in this game was.  I remember it being a lot scarier than that... This squash they meant is a sports game played in an enclosed court by 2 or 4 players who strike the ball with long-handled tennis rackets. Hope that helps you out. ^_~
