The flaming hallway represents the death Medic has seen in his work, all the patients that passed away under his care. The Pyro then reveals himself as the one who caused all of this death, the cause for all of the flames. The Medic vows revenge, and this fuses with the chaos of the Pyro, creating Uber Pyro.
