It's easy to remember his combos on any difficulty. He attacks with regular combo, draws back, takes a quick swipe and appears on your left, and repeats. At 1/4 loss of health, he does the air spin, which you guard against 3 times and reflect near the end. Otherwise it's all wails and action commands.

The entrance is in Radiant Garden outside of Ansem's study, where you first met Sephiroth in a cutscene. A new hole opens up there. Make sure you have most, if not all, your forms leveled up so you have the growth abilities. Glide is a must.
