  • Google: The Thompson Maneuver - it works like a charm for me - it's easy to do and alleviates 85 percent of my pain almost immediately - Orthopedist Alec Thompson popularized it - the directions come up when you Google it

  • I had a car accident in June of '05, in Nov. of '05, my lower back began to hurt so bad, that by Christmas all I could do was lay on the couch. Diagnositic nerve blocks showed pain coming from the medial branch nerves, but rhizotomy did not stop the pain. So, Dr. John Keating, does the SI joint have anything to do with my pain?

  • Does SIJ pain also cause pain in the medial branch nerves?

  • I had this surgery last august. Wow! Amazing Dr! I am pain free with no limitations. I'll answer any questions. Thanks for giving me my life back Dr. Keating. You rock!!!

  • My back is fucked up and I'm only 15:(

  • @lolScottXD aww i was 18 and thought that was young!!

  • if you have lower back pain go to a chiropractor or a P, its LESS invasive. I'm seeing a chiropractor right now for SIJ disfunction and trust me my back pain is going away.

  • I can nerver see a situation where fusion is necessary. Chiropractic and rehab should sort it.

  • Does SI pain feel like a hernia? I got it on my left side about under the hip by the left testicle. Pain shooting down leg, numbness sometimes in leg. Sometimes when I hang 1/2 my body off the table with 10-15 lbs weight on my bad leg it helps somewhat. This sucks, it's ruining my frign life. I'm not going to the doctor to pay a copay, to get to a specialist, to pay a copay, to get an MRI to pay a frign deductible. That's our wonderful HMO system folks.

  • @jamescaptainfarrell I have similar symptoms. Check to see if you have a sports hernia or possibly a labral tear of the hip....this sounds kind of like a labral tear of the hip..just shootin it out there for ya. Have you ever had a hip arthrogram?

  • @arfx3

The medical system wore me out. I'm not going for this. I'll deal with the pain. Doctor, $20, Referral, Specialist $20, Specialist > MRI = $500 deductible. Screw that.

Arthrogram? No idea what that is.

  • Hi N,

Thanks for getting back to me.

Yeah I've used lots of therapists, had prolotherapy, etc etc. Nothing giving long pain relief. I tested positive to two SIJ blocks, that relieved my pain, although very termporarily.

I take it you didn't like the idea of the general SI fusions in Australia and opted for iFuse and even flew to America to see the gentleman in the video above.

I'm in VIC, Melb and growing increasingly desperate.

    Good luck with the recovery!

  • informed consent????

no way would i have a surgical procedure on si.

    number one, stretching and nutrition.

number two, go to as many chiropracters and pt's as it takes.

  • I also have arthritis mainly at L5 vert. I am in a lot of pain but funny thing is that when I woke up from surgery I was in hardly any pain, probably due to the anaesthetic that was injected into the area etc. I hope to see my surgeon soon, so here's to find out what to do next!!!

  • Hi, I'm Lyssie and I am 21... I had left sided anterior spinal fusion T11-L3 for scoliosis when I was 17. They couldnt figure out what was causing my pain. Until a bone scan showed up some abnormaity in my left SI joint, I had injections and hallelujah they helped for a couple of weeks and this proved that the chronic pain was my SI joint! 3 and a half weeks ago (by the way my diagnosis is sacroiliitis) I had open denervation of the nerves that supply my SI joint. xxx

  • I believe that fuits and vegetables can reduce inflammation in joints!

I started taking Mona Vie Active for my shoulder pain and now I don't have to take Vicodin before I go to bed!

If you seriously want the Monavie Active, go to juiceberg's channel! They will hook you up!

  • Alkaline ionized drinking water is given by chiropractors to alleviate joint pain:

    bawellwaterionizers

  • Any type of surgery on the spine (yes, the SI joint is located at the very bottom of the spine) should be a last resort. In many instances, if you're having a problem with the SI joint, you will experience varying degrees of instability. This may be as minor as discomfort in your lower back/lumbar region, or as severe as barely being able to stand.

Personally, I prefer the chiropractic solution. A good chiropractor should be able to recognize this and address it adequately.

  • take calcium with vitamin d3 you'll feel much better.

  • does your ass go numb from SI pain? No matter the seat my ass goes numb and kills.

  • Also, if anyone has a list of US/Canadian surgeons/clinics who perform this surgery, I would appreciate it.

All my research indicates that the conventional view is held by all the surgeons here - that is "we only do disc fusion" Chiropractors diagnose this but they can only treat with manipulation. At least in my case this will not stabilize the sacrum.

  • In Australia who are the best surgeons for this surgery?

I need this surgery, 8 years of severe symptomatic pain and disability (almost completely unable to sit) after high impact snowboard fall onto left buttock.

  • A good pt can help get most of the pain out but medication is still better than surgery any day.You can always go back every 6 months or so if you need more pt.

  • I'm having SI pain from pregnancy hormones (relaxin). I don't have a lot of insurance for PT care but I have been doing the recommended exercises and wearing an SI brace. I started doing some chiro which had helped for a little while but now I'm feeling worse this week.... I'm supposed to keep going for a total of 12 weeks but I'm not sure if this is a good solution. I'm hoping once the pregnancy is finished this goes away!

  • Do not believe that chiropractors can help longstanding SI problems. You are much more likely to have the range of motion INCREASE, causing more pain in the long run. A knowledgeable chiropractor will tell you this little fact.

  • This surgeon fails to advize chiropractic care before submitting to this irrevesible procedure. CHIROPRACTIC!!!

  • The problem is many patients get worse with SI joint fusions. A better solution is a stabilization solution injected into a patient's SI joint's. I have a 90% success rate doing this for my patients assuming they have no prior surgeries. This is a permanent solution that still allow slight movement of a patient's SI joint. Do not have a fusion!

  • @janiga2001 What stabilisation solution do you inject into the si joint of these patients because I am considering this surgery.

  • @janiga2001 Hi Janiga2001, Could you please tell me what prolotherapy solution you inject into the si and the particular location you inject and how often for how many weeks? I have read that a particularly aggressive prolotherapy solution is required. I am in Australia with disabling intractable si joint pain - unable to sit down. I should put it to a doctor here before I head to the US to get the surgery done. Also should it be fluroscopically guided?

  • @n4979338 Hi n49, I have the same questions! I live in The Netherlands, have severe SI dysfunction for over 20 years. Had prolotherapy in Belgium, twice (in the dorsal ligamnets) , but without flurusc. guidance. The therapy had no effect. I am thinking of going to Germany for more prolotherapy but under flur. guidance and in other ligaments of the pelvis. Grtz from Amsterdam, Salsarosi.

  • @salsarosi Hi Salsarosi, prolotherapy for SI is bullshit. The best: foot orthodic, si joint belt I use a bregg low profile n thin compressor belts under, yoga stretching, meds like lyrica, baclofen, valium. Then surgery worth it, SI Bone iFuse procedure is the only reliable, not cages, not bone morph protein - all dangerous. iFuse started in Austria recently, USA 2 years ago. I am going to the USA in September to have iFuse, after 8 years of chronic pain and disability. check iFuse or email them

  • Hey mate.

How did your ifuse surgery go?

Can you keep us posted please?

  • @brokenvase1 G'day broken, Well it has been a tough ride, I had iFuse 3 weeks ago now bilaterally (unusual). I seem to have had a decrease in pain but it is hard to be more detailed at the moment, I am mostly in bed with post surgical pain and still some back pain. Another month I will be able to provide a better run down... If foot orthodics, si belts/braces, yoga, tens, massage, cold/hot, meds: lyrica, baclofen, valium, oxycotin, anti-depresents/inflamatories fail...it is worth doing iFuse

  • Has anyone had this surgery by Dr, Keating? And have the screws stayed in place or have they loosened within a couple of years?? I considering having it done. I've been through the injections and physical therapy. It helped for a little while. I just want my life back and to be able to get back to work.

  • After surgery you will wsddle like a duck. CHIROPRACTIC!!!

  • The SI joint can become misaligned and my physical therapist corrected it.

I had a auto accident that a car pulled out in front of me on a highway and I pushed on the brake so hard it jammed my SI joint out of place.

My SI joint tends to go out of place several times and when just stepping over a curb.

When I researched auto accidents and SI joint injury I found out this was common in this situation.

PT,good shoes, stretching, and anti flammatories help me the most. BJ

  • Before doing surgery, please let me try! See my videos ProPTRehab. When you strengthen the multifidi with the ATM2, you might find some of that instability disappear. Also, I evaluated using the McKenzie eval & you can r/o the L/S 1st.

  • For anyone suffering from lower back pain caused by the SI joint, try alot of massage therapy, chiropractic work, and lots and lots and lots of stretching to fix postural issues and overly tight muscles before you look into surgery. i was suffering for 5 months as a result of a weight lifting injury and am just now finding the right combination of self administered massage, self-adjustment, and stretching to achieve normalacy again. theres always hope!

  • SI Joint Surgery: over 85% of people who have it end up in MORE pain than before.

Just due to TIME alone the fusion will fail. The SI Joint needs to move. EVEN if it's a tiny amount. I mean Milimeters.

The good SI joint will eventually fail over time due to the odd movement caused by the Fused side.

And, only 3 to maybe 5 places in the US do this. NO medical Dr will treat someone for this only to prevent infection.

    Be an INFORMED patient.

Do look into this, but all of it.

  • I am so glad to see that there is a community out here and videos on this. I have had arthritis since I was 15, Im in my 30's now. I also have a ruptured L4/L5. Ive tried SI injections via floroscopic (the arthrogram he referred to at 3:09) guide and epidurals in the L4/L5 region. The latter of the two seemed to give me the most relief. I got it in May, it seems to have worn off. Of all the joints impacted by my arthritis the SI joint is by FAR the worse!! Thanks for posting!

  • After the surgery, is the patient typically able to return to a normal lifestyle and ability to exercise?

  • I'd also like to point out that many of us with SI joint problems seem to have other related problems...back, buttock, leg, foot pain.

I'm having a little bit of trouble understanding how the surgery presented here will help with the associated conditions. For me, all these pains seem to be related. Muscle imbalance? Out of alignment? Something!

  • I've been doing pilates for the last two months. It's helped with building the core. And I'm constantly trying to engage my core at all times while remembering NOT to let the pelvis tilt forward. Tried YOGA with various how to books.."Relief is in the Stretch" is NOT recommended. The recommended "Camel Pose" caused lock-up & severe pain!

I plan to continue with a life long dedication to pilates and perhaps prof. supervised yoga. Surgery is definately a last resort. But I'll keep it in mind.

  • Interesting!I've been suffering with a pain in the ass for 2yrs.1st they told me it was piraformis syndrome.6mnths ago the pain was from L4-L5 disc herniation.Had PT for the last two months, corrected my anterior pelvic tilt, now just finished up my 5th round of injections (epidural, facet joint,SI joint).Recent SI injection helped w/piraformis area pain. But I'm still in pain everyday from SI area to my little toes where the Morton's Neuroma is..Don't think screwing my SI joiny is goin to help me

  • My injury is a long story. basically i hurt my lower back playing sports and before it healed i injured it again...and again...and again. I went to physiotherapy and he says I have to put my pelvic alignment in place before I can start doing core strengthening exercises. He does this by pulling hard on my right leg. The problem is, my back is so weak that I constantly put my pelvis back out when I barely even do ANYTHING. it is very frustrating and I don't know what else I could do.

  • Just wondering if there are muscles or ligaments or tendons or something in there that are supposed to hold the SJ in place properly. If so, wouldn't it be best to work on those first, before undergoing surgery?

He does mention physiotherapy helping 73%, but what is going on with that 27% (over 1 in 4 patients) who don't get better with PT? Why don't they get better? Is there another option for them besides surgery? And do those who undergo surgery get total relief? Can they move freely?

  • You are correct my friend, explore other avenues before sugery

  • For a year I have been having problems with my SI joint. Just recently I discovered certain Yoga poses the are good for strengthening that area and relieving my pain. It worked for me. It is worth a try. This past year I have felt my SI joint pop when doing the littlest movements and I am in agony for days. The yoga really helped

  • I just came out of hospital with this pain, it is the most horrible thing ever happened to me. I was so depressed and in pain as any doctor was able to accurately diagnize the problem. 15 days at hospital without been able to walk. Finally back at home and starting to walk again.

  • Osteopathic Physicians (D.O),who can be Orthopedic Surgeons as well, are given extra training in musculoskeletal dysfunctions and they can probably help before someone will subject directly straight to surgery. I am not sure if Dr. Keating has an osteopathic physician on his team, but I would probably check it out.

  • The problem is most DO's dont use the manipulation they learn in medical school for problems like the SI joint, so you will probably be hardpressed to find one...Most of the DO's I know are interchangeable with MD's and probably wouldnt have a problem sending you to Dr. Keating for this type of procedure if all else fails...

  • You do realize that sij pain is just a symptom. Many people have sij subluxation without sij pain.

The pain can also come elsewhere as a result of painless dysfunction in pelvis. The pain can also come to the other sij, and often it does just that.

Often rotated ilium (sij subluxation) can cause piriformis syndrome or tight hamstrings, psoas, etc.

  • Ty for making this vid. I am currently Going through PT for this very problem. At this point I feel very little relief. The pain associated with the joint is aweful. The worst being a deep bone ache. No matter what postion I'm in I can not seem to find comfort;( I only wish my Doctor was a caring an informed as you. So thanks again

  • Yes, you are treating SIJ pain. But what is the cause of that pain? Most cases it is subluxation of SIJ on the other side. You can call it a blocked SIJ too.

SIJ pain is just one symptom of many possible that subluxated/blocked SIJ causes. I had ankle pain, tight piriformis and buttock muscles, tightness in upper back, etc. Not all have SIJ pain. And usually SIJ pain and low back pain are on the other side that blocked SIJ.
