I've seen the video footage of this accident, and the weight lifter did not move his feet. I could see NO hole in the floor any where near him. You should change the description of your slideshow, else post supporting evidence.

the glass had nothing to do with the lifter it was just to much weight or it was off balance.

It was an accident, it's not hard to see that the weight was not under his full control, maybe his hands were too far apart? I'm a nurse, and don't know a lot about weight lifting but can see why the elbow dislocated based on where the weight was and the position of his arm.
