Although I've found a way of playing it since I originally posted. I put a capo on the 2nd fret and play F, Am, F, G, and a C thrown in at the end of the chorus.

This is in fact how bon iver plays it, you can see the capo in the clip. However I reckon it goes to to C rather than Am on 'forever' and 'love you' in the chorus as well as at the end.
