Because McCain didn't say it 2 days after Clinton said she is a pitbull with lipstick. In fact I don't think she ever said she wore lipstick in any of her major speeches. Therefore McCain didn't call Clinton a pig, while Obama was clearly calling Palin a pig and his constituancy knew it. Look and listen to their reactions at the speech!

The difference betwene the two uses is, of course, that Palin had made a lipstick joke about herself less than one week before Obama used the lipstick pig cliche; he was either referring to Palin or made a stupid mistake of bad timing. When McCain used the lipstick pig cliche to refer to Hilary's plan (not HIlary herself), no such joke had been made previously.
