No speech writer would put such an idiom into a speech without carefully gauging its potential effect. And it doesn't take a rocket scientist to figure out what that effect would be. Note that they both got the same knowing laugh from the audience.

The difference betwene the two uses is, of course, that Palin had made a lipstick joke about herself less than one week before Obama used the lipstick pig cliche; he was either referring to Palin or made a stupid mistake of bad timing. When McCain used the lipstick pig cliche to refer to Hilary's plan (not HIlary herself), no such joke had been made previously.
