Common issue is the throttle linkage jams in the open position, then with an automatic bus a sizeable amount of power and a wide open throttle this happens.... driver can do very little...

As far as I'm aware and Im a bus enthuiast this bus had sticky throttle like any mini pointer dart does and it was unfortunate that it stuck at that time and went flying through
