The guy did not have his nuts removed. He went in to have his tonsils taken out and the surgery damaged his vocal cords so now he speaks with an unusually high pitched voice. The guest was obviously very offended and embarassed. If it happened to you, I can bet you wouldn't think it was nearly as funny.
