Had a legal prescription drug been found the toxicology would have listed the amount and registered it as "at therapeutic levels", but because there is no "theraputic level" for illicit drugs the toxicology listed the, low, amounts. Mitch didn't have enough heroin or cocaine in his system to kill him but the cocaine that was cut with what he thought was heroin likely contributed to his death.
Heart Failure 2005. That's what his death certificate and autopsy list as the official cause.
Correlation is not causation. Though I think that the cocaine that Mitch was not aware was in his heroin was likely the catalyst the truth is that there's no way to know whether on not Mitch's heart would have failed that night without drugs.
Wikipedia really needs to change the entry on Mitch's death.
