I believe in the end Buffy and Spike knew without a doubt that Buffy was in love with him. The scene where their hands are on fire to me is symbolic of their souls coming together. The moment their fingers were interlaced something happened. Spike looked at their hands...then Buffy's face became calm...with tears in her eyes she said "I love you". I think Spike's reply of "no you don't...thanks for saying it" was not convincing. Their eyes betrayed them...he knew it was the truth.
