Good job. Many of surgeons today prefers only an endonasal approach to frontal sinus. I had done trephination of frontal sinus only one time when I had a patient (15 years old boy with airgun bullet in left frontal sinus) when I done that under local anesthesia.
What I believe you are trying to say is that this procedure is obsolete. The vast majority of surgical procedures performed today are old & quite useful. Those without any knowledge of rhinology or sinus surgery would probably agree with your comment; however, this is the safest & most secure way of treating a severe, acute frontal sinus infection. After treating the acute disease, the patient may be taken back to the OR at a later date for an endoscopic approach.
Why do you open it from outside. we only to this to obliterate the sinus with abdominal fat.
a normal procedure would be done only endoskopically

where do they did this? ancient egypt?... i think there are other methods... this seem to be an old precedure.

Probably it is being done at a teaching hospital. An experienced surgeon would be correcting a resident as the resident performs the procedure.
