At the moment Jack died in the end, he was inmediatly brought to the "alternate dimension", where everybody forgot about what happened (but that alternate dimension was made by them to reunite with each other and move on... to heaven i suppose), but others like Hugo, Ben, Kate and anyone else who lived, continued their lives until they died for other reasons and reunited in that alternate dimension.
