But seriously, my old home town once had two of these diaphones for alerting the volunteers to respond to the station, (we had full-time county firefighters - but on ocassion, the full time crew had to go out of town). I wish I had recorded our old diaphones for posterity.

the i-force siren from american signal (formerly aca) has "airhorn" available for a tone, along with the usual variety of wails and yelps. i know of one department in southern NY that replaced their diaphone with an electronic air-horn.
i used to belong to a department that had a whelen five-cell stack and a diaphone. the whelen yelped at noon every day (except sundays), and for calls of course, but the diaphone is silent.
Could be that one has a brassie that's getting a little worn-out. Sounds almost like the one that Garden City Park (NY) had on Station 2 up to Oct 24, 1986. Only difference was that one had a pitch shift in addition to the "gargling" noise. No air regulator and a 34-foot pipe. Maybe this horn has a similar set-up. Whatever, it has a nice, sinister sound to it.
They need to repair that or get a new one. It is very sick sounding. :(

That's a worn piston alright, and probably also a brass one. I've never had an aluminum piston horn able to make that sound, only a heavy brass one.

I don't think its a matter of being clogged. My bet is that the piston in the diaphone is worn down. That is a relatively easy fix.
