Bolvar is at the top of the citadel, probably in the throne room.

Bolvar is alive for sure, and not an agent of the lich king. You can hear him yell at arthat at the beggining of ICC. Now this is just a my guess but in HoR Uther says that someone MUST take the place as the new leader of the scourge (lich king) I am guessing that it will be Bolvar or Mograine who accepts that place. Not sure thought just me guessing :P

Mal ganis IS the leader of the scalet onslaught he survived the attack by arthas and he disguises himself human to lead the scarlet onslaught you fight him in Onslaught Harbor but he survives the fight and legs it maybe to come back again as future boss
