Did some checking into this one . Turns out it was a union "electrician" that installed a garage door opener outlet with no ground. The cord on opener, became frayed exposing an ungrounded conductor and making contact with the rail. This made the entire door, frame and rail system "alive". The "electrician" also had a drinking problem that WAS addressed by the union.His name is Buddy Shea.
Electrical safety is one part of doing the job right.
Making sure that it was correctly mounted, to include a ground connection, is part of the job.
Being sure that it is plugged into a properly grounded outlet is another part.
Had both been done this would never have happened.
Installing equipment such as a satellite dish requires a ground for lightning protection, and there are no requirements for this type of installation except that provided.

General house inspections that banks require when someone buys a house don't guarantee that everything is safe. A house inspector is basically someone who has undergone a couple of months of training on inspecting every part of a house; and that is not enough time to make them an electrical expert. But some electricians have become house inspectors to bring in more income, and those people I would trust.
A permit is not necessarily going to solve tragedies like this one. There are many inspectors that aren't qualified to be electrical inspectors,that simply got their job through nepotism or simply because they will work for low wages.And even if an inspector is qualified, they still don't go around and check every connection and wire in a building. Also, electrician licensing doesn't guarantee work is going to be performed correctly,it basically means someone is insured and legal to do the work.
So basically what it seems happened here is that the ground wire was not connected on the garage door opener assembly, so the frame of the garage door was not grounded, and there must have been some shitty wiring there somewhere which allowed a hot wire to come into contact with the frame of the door, and when the kid was splashing water around all over the place he made a connection to ground when he touched the door in the water and it went right through him. This is why bonding is essential.
this just makes me angry they need to make a law electricians have to carry an electrical code book with them at each job so before they do shit like this they see what they should really do CORRECTLY!!!
Knew someone that had a contractor build a garage and use 14ga wire in bx and buried it underground, it was hook up directly to the service line in the meter box, no breakers. Ended up having to rip it all out for him and and ran 4 guage wire to his basement where the main breaker panel with a 60amp subpanel in the garage using aluminum conduit, also had to upgrade his sevice from the 60amp fuse breaker he had, which the wire was rotted out, to a new 200amp service 4/0-3 SEU Aluminum cable.
Permits do not guarantee the work is done right. No inspector is going to take the time to trace down the wires. You need to hire a qualified electrician, would have never happened. So sad.

we have the same tradie plague here now. I think it may have something to do with them trying to pay off impossible million dollar homes. they have no other choice than to cut corners.
Here in swe u dont get anything out on the ensurance if you dont have a paper on a certified doing the job. And btw we got 230V single phase and 400V on 3 phase.
If that 3 conducter wire that was shown, was the one used to power even a 110 volt garage door, especially ungrounded is just plain dumb. like a 20 gauge wire wow
It's sad that this has happened but people do this type of work all the time,homeowners as well as construction personal. Individuals need to have proper work ethics! Somebody knew that the work done in the video was incorrect.Inspected or not the installer is to blame! If he knew enough to use the ground wire to make it work,he knew enough that it was dangerous and wrong! Unfortunately this happens around the world,it's sad to to think the work was done to save some time and/or money.
I fixed my neighbors wiring because I received a small shock when grabbing the garage door track. They thought I was full of crap - until I had them touch it.
I've found in some homes that the ground wire was cut off at the sheath because the original outlet was only a two prong...and they didn't bother connecting the metal conduit box to it.

He should have used a 2 location RF switch instead of a three way.
But chances are he never heard of an RF switch. "No wires used batween the two RF switches."

I fixed my neighbors wiring because I received a small shock when grabbing the garage door track. They thought I was full of crap - until I had them touch it.
I've found in some homes that the ground wire was cut off at the sheath because the original outlet was only a two prong...and they didn't bother connecting the metal conduit box to it.
I hate stupid contractors. It only ends up costing the home owner more later, to correct for it.
